    CLINICAL PHARMACIST REMOTE

    About the Role

    We are seeking a licensed Clinical Pharmacist to support in a remote capacity. This role focuses on improving medication safety, optimizing therapeutic outcomes, and supporting interdisciplinary care teams.

    • • Review and verify medication orders for safety, accuracy, and clinical appropriateness.
    • • Conduct comprehensive medication reviews and identify therapy gaps.
    • • Provide evidence based recommendations to physicians and care teams.

    < p>You will work within a multidisciplinary team that includes physicians, nurse practitioners, registered nurses, case managers,and pharmacy support staff.The team culture emphasizes collaboration ,accountability,and measurable clinical results .Daily communication occurs through virtual meetingsand shared systemsRequirements < ulstyle =' list - style - type : none' > < li >& bull ; Doctor of Pharmacy degree required.< / li > < li >& bull ; Active Pharmacist license in good standing.< / li >
