Responsibilities:

The Licensed Practical Nurse (LPN) in Home Health delivers quality patient care and performs technical skilled care in patients' homes:

Provides services as per nursing practice laws, following the care plan and collaborating with the healthcare team.
Educates the patient's family on care and maintaining a healthy environment.
Monitors and reports the patient's response to treatment and condition changes regularly.

Clinical Competence:

Delivers high-quality clinical services within the scope of practice, following infection control procedures.
Reports the patient's nursing needs to the supervising RN continuously.

Documentation and Care Delivery:

Communicates effectively with physicians, patients, and family members about care needs.
Thoroughly documents patient visits in compliance with policies and regulations.

Quality:

Participates in performance improvement efforts to enhance patient outcomes.

Professionalism (timeliness, training, compliance):

Actively engages in meetings, including case conferences, always punctual.
Approaches work positively, ready to assist patients, and open to acquiring new skills.

Teamwork (on-call, productivity):

Follows the agency's Episode Management process.
Performs any other assigned duties effectively.

License Requirements:

Current LPN licensure in the state of practice.
Current Driver's License, vehicle insurance, and access to reliable transportation.
Current CPR Certification.
