In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart.

You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest.

To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.

ResponsibilitiesCoordinate with other colleagues to assess, plan, implement, or evaluate patient care plansDocument any changes in patient conditions and report on changesMaintain accurate records of patient care and discuss observations with the supervisorRN (Home Visit), RN is responsible to transporting completed client charts to the clients home within 7 days ofchart preparation to obtain the required signatured for the state of North Carolinas mandatory documents forthe below reasons:

Initial Admission to the home care agency Home Care Perfections90-day Supervisory visitsAnnual AssessmentsCare Plan Updates as neededRN (Agency Forms), RN is responsible for signing all required documents in the Employee charts upon hire, annual training and evaluations.

QualificationsCurrently licensed Registered Nurse (RN) in state of employmentPrevious experience as a Registered Nurse is preferredExcellent interpersonal and client care skillsEffective CommunicationTime ManagementValid drivers licenseFirst aid/CPR certification requiredBasic Comprehensive Computer Skills (Microsoft, Excel)Flexible work from home options available.
