    Job responsibilities:
    Screen for social determinants of health and incorporate findings into care plans as applicable.
    Work 40 hours per week with 8-hour shifts.
    Support patient and family meetings related to care planning and transitions as needed.
    Connect patients with community resources and support services as appropriate to identified needs.
    Facilitate care coordination across providers to support continuity of services.
