Responsibilities
- Provides competent nursing care & interventions to home health patients according to state and industry standards Supervises care provided by Home Health Aides Licensed Practical Nurses (LPN) assigned to the care team Completing supervisory visits of HHAs as required Helps maintain continuity of care by collaborating with the interdisciplinary team attending physician developing integrating managing patient care plan making referrals other disciplines needed Conducts field visits evaluate initiate start of care clinical outcomes monitoring follow-up performance improvement initiatives Utilizes accepted company teaching materials in patient/family education Assesses monitors documents reports progress patient's health condition using required documents clinician preferred technology established timelines Completes documentation timely accurately point time requirements accordance industry standards e.g OASIS assessments SOC/Admissions Recertifications ABNs subsequent visits physician orders coordination etc Honors patients/residents' rights notified advance work treatment services provided works patient team adhere physician-approved plan Ensures equitable treatment self-determination individuality privacy property civil rights Complies corporate compliance program Meets agency productivity standards utilizing time resources effectively efficiently Participates center/agency surveys licensure/Joint Commission mandatory in-services case conferences staff meetings programs e.g Performance Improvement Program Follow established safety regulations procedures report job-related functions/tasks occupational hazards necessary Maintains current license certification clinical records federal state agency guidelines Ability self-directed prioritize manage time appropriate meet outcomes strategic goals targeted deadlines
