- Chart closure within 72 hours
- Schedule = 18 PPD, minimum see 18 PPD
- Monday Friday (8 5)
- A mix of peds/adults (Will consider IM only)
- A decision is typically made within 7-14 days
- We will also accept DO
- BE will be considered if truly BE (5 years or less since residency)
- Active DEA needed before the start
- Credentialing takes 30-90 days (six weeks is common if the provider submit app./supporting documents timely)
- Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years)
- Primary series plus booster, regardless of the date received or 1 dose of the current COVID vaccine (2023/2024 formulation) Required
- HCO & Vendor agree to a 30-day notice for ending all assignments
- Patients Per Shift: 16
