    Under the direction of the UR Administrative Supervisor, this position is responsible to perform office duties such as: answering incoming calls, copying, scanning, faxing, filing and attend to walk-in members, staff and vendors, and support the discharge coordinators.

    The Discharge Coordinator Trainee will interact with other departments, clinic personnel, and outside providers in a professional and friendly manner, to create and maintain a positive relationship with our internal and external customers.

    1.1. Answering and processing incoming telephone calls in a timely manner utilizing a multi-line telephone system.
    1.2. Verify member eligibility using heath plan websites as needed
    1.3.

    Responsible for scanning of discharge summary plans, post hospitalization orders, member correspondences, andhospital records accurately and in a timely manner into NextGen EHR system.

    1.4. Initiate daily NextGen tasking to distribute post discharge plan orders to team.
    1.5. Assist with communicating to Case Managers and administrative staff in coordinating post discharge patient needs.
    1.6.

    Assist case managers with the ordering and delivery of DME to facilitate the patients leaving the facilities in a timelymanner.

    1.7. Assist in scheduling post discharge appointments on a daily basis.
    1.8.

    Assist in generating authorizations to appropriate providers with accurate ICD-10, CPT, or HCPC codes along with providerspecialty in accordance with post hospital discharge order, departmental and regulatory timeliness standards.

    1.9. thorough documentation in NextGen of actions taken to fulfill discharge plan orders as well as provider office and patientcommunications.
    1.10. Assist with mailing authorization, faxing of records to providers and mailing appointment letters to patients.
    1.11. Completing assigned tasks within departmental and regulatory standards.
    1.12. Adhering to departmental, CMS and health plan requirements in relation to generating authorizations, and notification ofdeterminations within regulatory requirements.
    1.13. Maintain filing and documenting into departmental systems in a timely manner.
    1.14. Adaptable to regulation and necessary procedure changes that affect departmental processes.
    1.15. Compliance with HIPAA regulations and maintain patient confidentiality.
    1.16. Maintain assigned duties timely and accurately. Communicate with Supervisor if there is a delay in completing duties, ifwork falls behind or if problems arise.
    1.17. Communicate in a positive, helpful, polite, respectful, and professional manner with fellow staff and outside vendors.
    1.18. Provide coverage, as necessary.
    1.19. Cultural and Linguistics training required annually.
    1.20. Other duties as assigned.


    Requirements

    8.1. High school graduate or GED certification, required.
    8.2. Medical terminology training or experience, preferred.
    8.3. Training or experience with CPT, HCPC and ICD-10 coding, preferred.
    8.4. Typing certification of 40 wpm, required.
    8.5. Proficient written and oral communication skills.
    8.6. Proficient organizational and time management skills.
    8.7. Demonstrate ability to follow through with delegated tasks and accountability. 8.8 Proficient in computers skills including but not limited to Excel and Word required


    The pay range for this position at commencement of employment is expected to be between $17.70 and $24.30 however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, skills, and experience.

    The total compensation package for this position may also include other elements, including a sign-on bonus, restricted stock units, and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.
