    Position Overview:

    The role of the Case Manager Social Worker involves being assigned to specific clinical care units or patient populations to deliver necessary services to individuals and families facing medical, psychological, or social challenges that may hinder compliance with treatment plans, access to suitable healthcare services, or timely discharge.

    Responsibilities include arranging support to address patient-specific needs such as placement, essential living resources, home health services, counseling, and post-discharge assistance to optimize overall outcomes. Collaboration with the Nurse Case Manager and other healthcare team members is essential to ensure comprehensive care.

    Additionally, the Social Worker serves as an advocate, guaranteeing the prompt identification and fulfillment of patients' physical, psychological, and social requirements while upholding their rights.


    Experience Requirements:
    Minimum of 2 years experience in Clinical Case Management or relevant position


    Education Requirements:
    Master's Degree in Social Work


    Licensure/Certification Requirements:
    Preferably Licensed Clinical Social Worker Or Certified Case Manager


    Hours of Work:
    Monday - Wednesday, from 7am to 7pm


    Weekend Availability:
    Occasional, as needed


    On-Call Duty:
    None
