- Patient Assessment: Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs to develop individualized care plans.
- Care Planning: Collaborate with the healthcare team to develop and implement holistic care plans, setting realistic goals and objectives for each patient.
- Clinical Care: Provide direct nursing care, including administering medications, wound care, and monitoring vital signs, while adhering to established protocols and guidelines.
- Rehabilitation Support: Assist patients in achieving their rehabilitation goals by providing physical assistance, mobility training, and therapeutic interventions.
- Patient Education: Educate patients and their families on their conditions, treatment plans, and self-care techniques to facilitate a smooth transition from the hospital to home.
- Multidisciplinary Collaboration: Work closely with physical therapists, occupational therapists, speech therapists, and other healthcare professionals to coordinate and optimize patient care.
- Patient Advocacy: Advocate for the rights and preferences of patients, ensuring they receive compassionate, ethical, and culturally sensitive care.
- Documentation: Maintain accurate and detailed medical records, including patient assessments, care plans, progress notes, and discharge summaries.
- Safety and Infection Control: Adhere to infection control protocols and safety measures to minimize the risk of infection or injury within the rehabilitation unit.
- Continuous Improvement: Stay current with the latest developments in rehabilitation nursing practices and contribute to the improvement of nursing protocols and procedures.
- Valid Registered Nurse (RN) license in state of practice.
- Bachelor of Science in Nursing (BSN) preferred.
- Previous experience in rehabilitation nursing is a plus.
- Basic Life Support (BLS) certification required; Advanced Cardiac Life Support (ACLS) certification may be required for some positions.

Job Summary:As a Registered Nurse specializing in Rehabilitation, you will play a crucial role in the recovery and well-being of patients undergoing various rehabilitation programs. You will work collaboratively with a team of healthcare professionals to provide high-quality care, support, and education to patients and their families. Your primary responsibility will be to assess, plan, implement, and evaluate nursing care within the rehabilitation setting, ensuring the best possible outcomes for patients.

VT or multi-state license
