    Overview



    We are a mission-driven, service-orientated industry leader looking for an ACT Program Liaison. You will be working on an interdisciplinary Assertive Community Treatment (ACT) clinical team working with individuals diagnosed with a Serious Mental Illness or General Mental Health diagnosis under Court Ordered Treatment. This individual will support the ACT team out of our integrated outpatient clinic in Tempe, AZ.

    Responsibilities



    Seeking a Full-Time ACT Program Liaison for our Priest Drive Integrated Health Center in Tempe, AZ.

    Full-Time Employed: 40 hours/Monday - Friday

    Location: Priest Drive and Broadway Rd. in Tempe, AZ

    Bilingual in Spanish is a plus (Additional Language Differential Pay Available)

    $2/hour ACT Differential

    This role provides daily administrative support for the ACT Clinical team in compliance with all program requirements per SAMHSA guidelines, Western Interstate Commission for Higher Education (WICHE), Mercy Maricopa and others as identified by Terros. This individual will liaison with clients' prescribing Clinician, Nurse and other members of the clinical team to ensure the timely development and implementation of the individual service plan. This position provides support to the ACT Clinical team as requested by the ACT Clinical Coordinator, Clinical Director or Site Administrator.

    Qualifications

    • Minimum of High School Diploma, GED required; Associates degree in medical and healthcare (e.g. healthcare administration, insurance billing and coding, psychology) social work, human services, communication preferred.
    • Minimum of one (1) year experience in behavioral health or related field required.
    • Minimum of two (2) years' office experience and successful interaction with all levels/disciplines of people in the work environment . Experience working in clinical/behavioral health setting preferred.
    • Ability to maintain professional working relationships.
    • Certified Psychiatric Rehabilitation Practitioner (CPRP) certificate (preferred)
    • Intermediate skills in Microsoft Word, Excel, and Outlook required. Experience in PowerPoint and electronic medical record - NextGen preferred.
    • Experience with multi-line phone systems and ability to perform general math skills required.
    • Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employment
    • This role is a non-driving position. This position is performed at one location and does not require travel to various Terros Health centers. May be 18 years of age and with less than two years' driving experience or no driving experience.
    • Must pass a TB Test
