    Description

    Job Summary

    Responsible for assessing, planning, implementing, and evaluating nursing care within a skilled nursing setting. Ensures continuity of care for residents by providing direct care, supervising CNA's, LPN's, and support staff. Participates in resident and family teaching sessions. Acts as a resident advocate, focusing on the facility's mission.

    Core Job Responsibilities

    • Assesses residents on admission, readmission, and when conditions change. Documents accurately in medical records.
    • Informs physicians of any changes in residents' conditions and ensures follow-up actions are taken.
    • Provides detailed shift reports and participates in walking rounds.
    • Evaluates and manages pain, educates residents and families on pain management.
    • Delivers resident care in a safe environment, supervises staff to prevent errors.
    • Develops and implements care plans based on assessments and team goals.
    • Supervises and guides healthcare team members to execute the care plan effectively.
    • Coordinates staffing for current and upcoming shifts.
    • Administers medications following safety protocols and reduces errors.
    • Acts as a leader for CNA's and LPN's, promoting teamwork and department excellence.

    Education/Experience Requirements

    Required: Minimum two years of experience in medical/surgical or nursing home settings. At least one year in a supervisory role.

    Licensure/Certification Requirements

    Preferred: Registered Nurse licensure in NYS/BSN or related degree preferred.
