The Behavioral Health Care Manager will be responsible for reviewing patient charts and communicating with the Clinical department and providers about any irregularities as part of chronic care management
Responsibilities:
Provides assessment and care management services, including: The administration of validated rating scales Initiates behavioral health care planning concerning behavioral or psychiatric health problems Revises / modifies care plans for patients not progressing or whose status changes Brief psychosocial interventions if needed Ongoing collaboration with the billing practitioner Maintenance of the registry/tracking sheet CConsultation with the psychiatric consultant Has a continuous relationship with the patient.
