    Nurse Care Manager

    Role Summary


    The role of the Nurse Care Manager is responsible for assisting patients diagnosed with complex illnesses, recovering from a traumatic clinical event, or managing multiple clinical co-morbidities.

    The NCM leads the development and ongoing management of a comprehensive, individualized care plan with each patient. Routine telephonic assessments clinically support the patient.

    The NCM supports patients by providing scheduled telephonic clinical assessments and referral to Narus care coordination for non-clinical needs:


    • Perform appropriate assessment and triage of patient calls.
    • Identify clinical needs and create appropriate care plans to support patient.
    • Manage issues identified by the patient care plan
    • Conduct preventative telephonic assessments to screen for new issues, reassess the patient, update the care plan, and evaluate progress toward the goals.
    • Evaluate changes in patient-reported symptoms and conduct additional triage and screening to determine next steps.
    • Support referral to their primary care provider or hospital when clinically necessary.
    • Educate patients and families on treatment options, including cost, efficacy, and side effects

    Job Requirements


    • Excellent time management, flexibility, and organizational skills with ability to work independently
    • Active, unrestricted RN license in the state(s) of practice -Multi State license required
    • A minimum of two (2) years of related clinical experience, telephonic care management experience desired.
    • Proficient on Microsoft Office programs, ability to document while assessing the patient telephonically.
    • Highest degree of empathy and compassion with excellent ethics
    • Ability to analyze patient claims data, draw conclusions, and act on that information in a timely manner

    *Pharmacy Tech experience preferred

    *Preferred but not required experience- CCM-Case Management-Pediatrics, Oncology
