- Provide comprehensive case management services for individuals currently in care or former program participants living independently.
- Coordinate with county social workers, foster parents, probation, and other community agencies to develop and implement transition and housing plans.
- Contribute to the development of independent living skills curriculum and parenting groups.
- Plan and facilitate bi-weekly program management meetings.
- Offer housing and life skills support to program participants as needed.
- Market and recruit program services, as well as compile and submit program reports/data.
- Bachelor's degree in social services or related field, accompanied by at least one year of experience in youth work, life skills training, and group facilitation.
- Valid driver's license, acceptable driving record, reliable transportation, valid insurance, and a clear Motor Vehicle Check.
- Demonstrated strong organizational skills.
- Ability to maintain focus and attentiveness for extended periods.
- Skilled in developing, guiding, and providing follow-up for client transition plans.
- Capable of promoting a positive attitude towards LSS and other programs to the public.
- Familiarity with community programs and referrals.
