    Behavioral Health RN Case Manager position at Elara Caring:

    Position:
    Behavioral Health RN Case Manager ($5K Sign-On Bonus)


    Location:
    New Bedford/Fall River


    Schedule:
    Monday through Friday mid-shift (afternoon/evenings)


    Responsibilities:
    Assess the assigned caseload of behavioral health care patients and families to identify their physical, psychosocial, and environmental needs. Assume primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment.

    Communicate significant findings, problems, and changes in health conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.

    Revise the plan of care as needed and notify relevant team members. Counsel, instruct, and involve patients, facilities, and families in following the Interdisciplinary Plan of Care. Teach behavioral health philosophy and pain and symptom management to staff in alternate care settings. Initiate, supervise, and document observance of the behavioral health aide and LPN delivery of care per agency policy. Provide appropriate support at the time of death and perform bereavement assessment.


    Requirements:
    Current, unrestricted state RN license. 1+ years of experience in a clinical care setting as a nurse. Able and willing to travel within branch/office coverage area. Must have a dependable vehicle, valid driver's license, and current auto insurance. Must be able and willing to travel 50%. Associate's Degree or Bachelor's Degree in Nursing is preferred. Previous Home Health or Hospice Experience preferred. Previous experience with Homecare HomeBase preferred.
