- Assume the administrative authority, responsibility, and accountability of implementing our medical services, policies, and procedures according to federal, state and local regulations; and current practice of post-acute and long-term care medicine.
- Coordinate medical care, including emergency treatment procedures, maintain effective liaison with attending physicians, and implement methods to keep the quality of care under constant surveillance.
- Provide necessary physician services for those residents within the skilled nursing facilities and/or clinic environment
- M.D. or D.O. required
- Gerontology certification preferred
- MD or DO license in State of Indiana
- ABMS Board Certified in: Internal Medicine, Family Practice, Geriatrics or Hospice/ Palliative
- 5+ years of experience working with geriatric or long-term care population
- Demonstrated experience working with and educating NP's
- Health care management/leadership experience
- Familiarity with CMS, Medicare Advantage, SNP Rules and Regulations
- HCC and Risk Adjusted reimbursement methodology
- Inpatient/SNF utilization metrics and bed day management
- State specific collaborative practice agreements and midlevel practitioner licensing
- Knowledge of EMR
