    Position Summary
    This position assists with the development, implementation, evaluation, and ongoing improvement of PrimeCare's early childhood development programming.

    Specifically focused on PrimeCare's primary care pediatric clinic, this position is an integral part of the care team providing training to staff, new clinic programming in positive early childhood development, and direct services to high-risk children, aged 0-5, and their families.

    Duties and Responsibilities

    Complete needs assessment to determine service needs of high-risk pediatric patients, 0-5 years of age, related to developmental issues and parenting skills.

    Directly manage and supervise early childhood development staff.
    Create and manage workflows related to early childhood screening, treatment, and access to treatment and services.
    Establish referral linkage with external service providers through community outreach and engagement.

    Identify, and track, high-risk early intervention pediatric patients in collaboration with the Early Intervention Navigator to ensure family education is provided, referrals are processed, and service is rendered.

    Promote clear communication amongst team members regarding high-risk patient referrals, development of care plans, and progress to meeting treatment goals through:

    Collaboration with treatment team to establish and optimize roles, responsibilities, and success measures,
    Leading multidisciplinary case review on high-risk early childhood patients, when appropriate.

    Develop curriculum and lead family groups related to early child development and parenting skills.
    Provide early childhood development education and training to PrimeCare and Ascension residency staff.

    Collaborate with Health Center Medical Director and Director of Clinical Services to develop and implement early childhood development protocols, policies, and procedures.

    Monitor and gather program outcome data.
    Collaborate with grant management staff to complete grant reporting.
    Represent PrimeCare in external settings related to early childhood initiatives.
    Collaborate and participate in the selection, development, and maintenance of patient education materials.
    Participate in PrimeCare's quality processes and activities, as indicated.
    Maintain current knowledge of regulatory agency requirements of compliance and ensure that those standards are practiced in the clinic.
    Other duties and responsibilities, as assigned.

    Required Knowledge, Experience, or

    Licensure/Registration
    Bachelor's degree required in addition to 5+ years of applicable early childhood development experience. Master's degree in Early Childhood Development or similar degree preferred.
    3-5 years of experience in an early childhood setting.
    Prior experience working with an underserved and under-resourced population.
    Interest in the primary care setting and being a part of a multidisciplinary team.
    Proficient use of Microsoft suite of products, including Excel.

    Physical Demands
    Must be able to remain in a stationary position 50% of the time.
    Must be able to move around the clinic site 50% of the time.
    Constantly operates a computer, computer printer, copy machine, and telephone.

    Occasionally positions self to maintain exertion of physical strength to move objects of 10 pounds from one level to another.

    Must be able to transport from one site to another.
    Must be able to cover other shifts, as necessary.
