- Interpret and implement Agency's philosophy to staff and the community.
- Collaborate with hospital teams to assess patient's needs and establish discharge plans.
- Educate patients on illness, home environment, and support systems.
- Assess comprehension of procedures and treatments.
- Review referrals and assess service criteria.
- Explain agency services and policies to patients and families.
- Verify insurance coverage and financial responsibilities.
- Refer patients to other services if needed.
- Coordinate medical supplies and equipment needs.
- Provide feedback post-discharge to healthcare facilities.
- Participate in staff orientation and improvement activities.
- Evaluate and set performance goals.
- Current NY RN license.
- Preferably a Bachelor's degree in Nursing.
- 1-2 years of community health experience.
- Effective problem-solving and prioritization skills.
- Familiarity with Medicare, Medicaid, and insurance regulations.
- Strong interpersonal, leadership, and documentation abilities.
