- Preferred Schedule: Mon-Fri, 8:30am to 5:30pm
- Worksite Setting: FQHC - outpatient
- Scope of Work: Primary Care, must be comfortable seeing all ages
- Licenses, Certifications, Requirements: Active, unrestricted IN license, DEA, BLS, 1-2 years FM exp preferred
