    Description

    JOB SUMMARY
    The Care Coordinator Nurse integrates care and services with an emphasis on preventative care and education.

    The Care Coordinator Nurse works collaboratively with patients, families, physicians, and other members of the health care team to achieve the highest quality clinical outcomes with the most effective use of available resources.

    The Care Coordinator assumes responsibility for identifying high risk patients and collaborates with the Senior Care Coordinator to enroll the patient into the appropriate level of care coordination.

    The Care Coordinator is knowledgeable about various age related care protocols and functions with the scope of their practice. The Care Coordinator will work with members of the wellness program to coordinate preventive care.

    Qualifications


    JOB QUALIFICATIONS

    EDUCATION:
    Graduate of an accredited Licensed Practical Nurse Program (LPN)


    Licensure:
    . Registered LPN License; AHA Healthcare Provider


    BLS

    EXPERIENCE:
    Preferred experience in a clinical or hospital setting. Previous wellness/ health coaching experience desired


    JOB SKILLS:
    Knowledge of clinical patient care. The ability to use critical thinking skills and Highly organized with the ability to maintain accurate documentation and reporting. Demonstrates evidence of exceptional leadership, communication and collaboration with other health care team members. Maintains core values that are consistent with patient/family- centered care.
