    Supportive & Palliative Care is a team-based medical specialty focusing on comfort, care and planning for patients facing serious illness and their families. Supportive & Palliative Care team members help determine a care plan that meets your wants and needs, including pain management, symptom management, counseling and spiritual support, and other therapies. Baylor Scott and White is looking to add a Supportive & Palliative Care Nurse Practitioner to their Irving location. Bilingual Spanish/ English speaking candidates are preferred.

    JOB SUMMARY

    The Supportive & Palliative Care Nurse Practitioner (AGACNP) is a licensed, board certified, non-physician healthcare provider who actively renders direct patient care and possesses delegated prescriptive authority, is credentialed and privileged, and whose service is billed as an independent billing provider, as recognized by Medicare and Medicaid within the healthcare systems. The Nurse Practitioner provides assessments, disease and treatment education, and symptom management to meet the needs of palliative care patients and their families. The Supportive & Palliative Care Nurse Practitioner helps the patient and family maintain their maximum level of comfort and coping.

    Our Ideal candidate will have a minimum of 2 years of experience in direct palliative care as an NP and be bilingual English/ Spanish

    ESSENTIAL FUNCTIONS OF THE ROLE

    Schedule Monday- Friday Days

  • Symptom Management
  • Performing goals of care discussions with patients and/or family members
  • Advance care planning
  • Pain Management/Relief
  • Assesses the physical and mental condition of patients by performing and obtaining comprehensive or problem-focused physical examinations and medical histories
  • Prescribes, recommends and/or employs medical drug therapies or other forms of related therapeutic treatments and procedures or performs indicated procedures in accordance with approved protocols/policies with attention to efficacy, safety, and cost
  • .Formulates and implements comprehensive outcomes-based patient treatment plans based on patient assessments, evidence-based medicine, standards of care, professional practice guidelines and in partnership with physicians.
  • Counsel patients and families regarding medical processes, management of the health illness/condition, medication, nutrition, and health promotion.
  • Counsel patients about drug regimens and possible side effects or interactions with other substances such as food supplements, over the counter medications and herbal remedies.
  • Coordinate and collaborate with interdisciplinary specialty teams.
  • Coordinate and collaborate with other inpatient and outpatient medical specialty service lines.
  • Maintains appropriate records detailing the patient's treatment plans and outcomes.
  • May be required to perform patient care duties beyond regular schedule based on coverage needs of the department.

  • Experience with EPIC is a plus
  • Excellent collaboration skills
  • Ability to develop and implement comprehensive outcomes-based patient treatment plans
  • Ability to counsel patients concerning medical and psychological/psychiatric conditions, treatment plans, and behavior modification
  • Ability to interpret and evaluate laboratory and other diagnostic tests

    QUALIFICATIONS

    EDUCATION -Graduate of an Accredited Nurse Practitioner Program

    EXPERIENCE - 2 years AGACNP Experience Preferred

  • Current Texas RN and APRN license
  • Current Board Certification
  • RN- ICU experience preferred for new graduates
  • Bilingual English/ Spanish Preferred
  • CERTIFICATION/LICENSE/REGISTRATION

  • Acute Care Nurse Practitioner Certification
  • Texas Registered Nurse (RN) and Advanced Practice Registered Nurse (APRN) License
  • Basic Life Support (BLS) within 30 days of hire or transfer
