    We are seeking an Home Health LPN/ LVN to join our team You will be responsible for delivering high quality care to assigned patients.

    ​


    Job Summary:
    We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Primary Care
    Management team.

    The LPN/LVN/ in Chronic Care Management plays a vital role in providing high-quality healthcare services to patients and assisting in the coordination and management of their care.

    The ideal candidate will have strong clinical skills, excellent communication abilities, and a passion for
    patient-centered care.

    Responsibilities:

    Patient Assessment:
    Conduct thorough patient assessments to gather health information,
    including vital signs, medical history, and current medications, to assist in developing
    individualized care plans.

    Care Coordination:
    Collaborate with healthcare providers, including physicians, nurse
    practitioners, and other members of the healthcare team, to ensure seamless and coordinated
    patient care.

    Health Education:
    Provide patients with information on disease management, preventive care,
    and healthy lifestyle choices. Offer guidance on managing chronic conditions and promoting
    overall wellness.

    Patient Advocacy:
    Act as a patient advocate, ensuring that patient preferences and concerns are
    heard and addressed during the care process.

    Documentation:
    Maintain accurate and up-to-date electronic health records (EHR) for all
    patients, documenting assessments, interventions, and patient progress.

    Communication:
    Communicate effectively with patients, their families, and other healthcare
    team members to facilitate open and transparent communication regarding the patients care
    plan.

    Infection Control:
    Follow established infection control protocols and maintain a clean and safe
    clinical environment.

    Quality Assurance:
    Participate in quality improvement initiatives and contribute to the
    enhancement of patient care processes.


    Qualifications:

    • Current and valid LPN/LVN Compact license.
    • Graduation from an accredited LPN/LVN program.
    • Strong clinical skills and knowledge of primary care principles.
    • Excellent interpersonal and communication skills.
    • Ability to work effectively in a team-based healthcare environment.
    • Proficiency in using electronic health records (EHR) and healthcare software.
    • Compassion and dedication to providing patient-centered care.
    • Attention to detail and the ability to maintain accurate records.
    • Willingness to adapt to changing healthcare protocols and technologies.
    Preferred

    Qualifications:

    • Knowledge of chronic disease management.
