    Position Details:


    • Employment Status: Full-Time
    • Shift: Weekends

    Position Summary:


    The LPN Personal Care Unit Care Coordinator collaborates with the patient and family, care team, payers, and external entities to enhance the overall physical, mental, and psychosocial well-being and ensure a smooth transition from the facility to the next provider or care setting in compliance with relevant laws, regulations, and Life Care standards.


    Educational Background, Experience, and License Requirements:


    • Graduated from an accredited LPN program
    • Currently licensed/registered in the applicable State. Must maintain an active Licensed Practical/Vocational Nurse (LPN/LVN) license in good standing during employment.
    • Preferably, one (1) year of geriatric nursing experience
    • CPR certification required upon hire or should be obtained during orientation. CPR certification must be kept current throughout employment.

    Specific Job Needs:


    • Profound knowledge in the field of practice
    • Capable of making independent decisions when necessary
    • Familiar with practices, procedures, and the regulations governing operations in the post-acute care facility
    • Implement and interpret department programs, goals, objectives, policies, and procedures
    • Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation
    • Maintain professional relationships with associates, vendors, etc.
    • Ensure confidentiality of proprietary and confidential information
    • Adhere to company policies, including harassment and compliance procedures
    • Uphold integrity and professionalism by following Life Care's Code of Conduct and completing mandatory Code of Conduct training and other relevant compliance training

    Key Responsibilities:


    • Chart patients' information accurately and in a timely manner
    • Manage and coordinate patient care and services to help patients achieve or maintain optimal physical, mental, and psychosocial well-being
    • Administer medication accurately as per physician's orders
    • Lead emergency responses for patient and staff safety
    • Coordinate patient care plans and services with RN
    • Provide exceptional customer service and maintain a positive attitude towards patients
    • Assist in patient evacuations
    • Maintain regular and reliable attendance
    • Utilize reasoning skills, good judgment, and concentration effectively
    • Collaborate and communicate well within an interdisciplinary team
    • Perform various physical tasks during working hours
    • Read, write, speak, and comprehend the English language proficiently
