Summary
The Cardiac Rehab Team at Medical Plaza One in Fort Myers is seeking an experienced RN to join their team This is part time position working approximately 20 hours per week.
Responsible for the delivery and documentation of care delivered to Cardiac Rehab Phase I inpatients and Cardiac Rehab Phase II and Phase III outpatients// as appropriate to services provided at specific location// through assessment// diagnoses// planning// implementation and evaluation.
clinician// teacher// researcher// leadership and professional development.
Responsible for directing and coordinating care for Cardiac Rehab patients based on established Cardiac Rehab practice standards subscribed to by the American Association of Cardiovascular and Pulmonary Rehabilitation.
Demonstrates proficiency in Cardiac Rehab core competencies. Possesses knowledge of Cardiac Rehab guidelines// standards and reimbursement. Collaborates with other professional disciplines to ensure effective and efficient care delivery and the achievement of desired patient outcomes. Supports the organization's vision and mission. Uses knowledge of patient's age and cultural diversity in the provision of care.Contributes to the provision of quality care through excellence in customer service and performance improvement techniques that demonstrate positive outcomes in patient care.
