Clinical Supervision for licensure
The primary purpose of this position is to conduct home-based case management services for families. Develops, coordinates and supervises family service plans, assesses and identifies family needs and connects family to available kinship resources.
Develops appropriate family service plans.
Makes appropriate referrals to community resources.
Maintains records of all services provided and documents all assessments, meetings, decisions and reports.
Help families with paperwork to obtain guardianship.
Bachelor's degree in Social Work or 18 units of Psychology, Sociology and/or Communication and a minimum of two years of experience in a case management position or equivalent experience.
