The Nurse Navigator (RN) is an integral member of seasoned professionals that work with patients to navigate total joint replacement and spine surgeries preparing them for the surgical experience across the continuum.
Develop standardized care plans in conjunction with the operating surgeon.
Meet with patients one-on-one (in person or telephonically) to communicate the role of the Navigator in the care continuum and outline expectations for the patient throughout the episode of care.
Identify and track all pre-operative requirements for the patient and ensure completion.Establish and maintain relationship-based care with the patient, family and support system.
Coordinate the collection of clinical information from the patient record (EMR).
Perform telephonic or in-person health assessments to determine risk stratification (Including a comprehensive assessment of the physical, emotional, environmental, and psychosocial needs of the patient).
Utilize EPIC and other software systems in daily activities to effectively manage TJR and Spine patients throughout the continuum of care.
Learn and understand the various evolving requirements of insurers and regulators in relation to Total Joint Replacement and Spine Care.
Collect and organize all appropriate pre-operative information for all patients including but not limited to Outpatient PT assessment, pre-op labs, EKG, H&P, Chest X-ray, MRSA nasal swab, and pre-procedure discharge planning.
Collaborate with patient's PCP offices and/or hospital pre-op testing centers when necessary to gather important pre-op health information.Verify patient scheduling and completion of TJR and Spine education classes.
Identify and communicate barriers for patient's enhanced discharge planning.
As part of the Total Joint Replacement and Spine Program the Nurse Navigator is responsible for managing all assigned total joint replacement and spine patients throughout their preop episode of care, ensuring patient understanding of preop instructions, joint class, surgical site infection prevention measures and what to expect day of surgery.
Adhere to the scope of practice for a Registered Professional Nurse per Virginia state regulatory guidelines.Other duties as assigned by Manager.
Position Requirements
Graduate from an accredited school of nursing
Current registered nurse licensure in state of Virginia
BLS
Proficient computer skills and the ability to work independently, multi-task, and problem solve
10 CEUs annually
Excellent customer service and interpersonal skills
Professional Oral and Written Communication Skills
At least 2 years as an RN with preferred Orthopedic experience.
Telephonic case management experience preferred
Physical Demands:
Exposure to blood and body fluids, communicable diseases, chemicals, radiation, and repetitive motions
Pushing and pulling heavy objects may be required
Full range of body motion including handling and lifting patients as needed
Manual and finger dexterity
Hand and eye coordination
Sitting for long periods of time
Lifting and carrying items weighing up to 50 pounds as needed
Corrected vision and hearing to within normal range
Working under stressful conditions
