Responsibilities

Matrix Provider – What to Anticipate:

• Perform Adult/Geriatric Assessments, medical history reviews, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and precise documentation following Matrix guidelines and protocols
• Collaborate with physicians, case managers, social workers, family members, primary caregivers, and any relevant ancillary medical staff as necessary
• Engage with Primary Care Physician (PCP) on patient education, offer follow-up
• Deliver services in various settings including: Home Visits, Skilled Facility Visits, and Telehealth Visits

Type: On Call Basis/Part Time - Flexible-Nurse Practitioner

COMPENSATION: $90.00 or $115.00 depending on county per completed Assessment

Location: Home, SNF, Telehealth and other community settings in Central Square, New York

Hours: Days, Evening, Nights, weekend, Flexible Scheduling

Benefits: On Call Basis/Part Time providers are non-benefited apart from 401(k) and limited Voluntary Benefits

Qualifications

Essential Requirements:

• Master's Degree or equivalent experience and successful completion of NP licensure
• Master's Degree or equivalent experience and successful completion of PA licensure
• Current RN and NP or PA licensure in state of practice including prescription authority or eligibility to obtain prescriptive authority
• Board certified by the NCCPA, AANP, ANCC or the AACCN in a Matrix endorsed specialty
• Possess current BLS, ACLS or CPR certification
• Minimum of 1 year experience as a Nurse Practitioner/Physician Assistant. Ability to travel within or outside the state
• Demonstrate proficiency in computer skills and familiarity with EMR software
• Display exceptional verbal and written communication capabilities with patients, clients, and colleagues
• Exhibit a positive and sociable demeanor, and be a collaborative team member. Adapt well to frequent changes

Travel Requirements:

• Ability to travel, possess a valid state driver license, and capable of driving, provide proof of sufficient automobile insurance coverage for the state of residence
• Travel necessary to meet patients in their residences
