    JOB SUMMARY:


    The Unit Coordinator under the direction of Clinical Director/ Nurse Manager, will support the administrative functions on the assigned unit(s) including, however, not exclusive to business operations, clerical functions, rounding and visiting patients, and staff rounding in accordance with the standards, policies, and practices of Valley Presbyterian Hospital.

    These services will further the mission of providing high quality, cost effective healthcare for the people in the communities we serve and support the values and vision of Valley Presbyterian Hospital.


    EXPERIENCE/QUALIFICATIONS:
    Must demonstrate technical competence, professional clinical judgment, communication skills and sensitivity for the patient and families
    Computer knowledge for operations of word processing, Electronic health Record, intranet, internet, power point presentations, and simple spreadsheet functions
    Prior experience in clerical/general office, including filing, sorting, records maintenance, and chart sorting
    Strong customer service skills required to communicate with patients, departmental internal/external clients, staff, and physicians
    Fluency in Spanish and must pass the medical translation test within 6 months of hire/transfer


    EDUCATION:
    High School diploma or equivalent


    LICENSURES/CERTIFICATION:
    Must successfully complete and maintain LA City Fire Card certification at the time of hire or within the first 30 days of employment


    DUTIES AND RESPONSIBILITIES

    Performs daily patient and staff rounding and submit report to Clinical Nurse Manager and/or Director.
    Prepares communication mediums including phones and translating services ready to be in use by staff.

    Updates patient census board frequently, checks email daily, and post flyers and important education materials in the education binder in a timely manner.

    Responds to all codes in the unit with a phone ready to assist or call responsible parties upon request from person in charge.

    Educates patients and their families about Patient Portal and how to log in.

    Follows up in advance of the required due dates, with staff on expiring certifications, licensures, and annual health updates and maintains records of these requirements.

    Organizes sending thank you cards to discharged patients within 24 to 48 hours after discharge.
    Coordinates daily work flow, as well as the accurate and timely production of reports and requests.
    Maintains excellent interpersonal skills with other hospital departments, where applicable for smooth information flow and knowledge of system changes.
    Maintains inventory of unit supplies adhering to budget guidelines.
    Assumes active role in unit by participating in staff meetings.
    Attends mandatory in-services and some other professional education opportunities to enhance skills.
    Perform audits on various nursing sensitive indicators and quality measures upon request by Clinical leaders

    Participates in all departmental specific training, Environment of Care (injury/illness prevention, fire/life safety, hazardous materials, emergency preparedness, utilities management, medical equipment management, safety and security management), infection control (standard precautions, TB Exposure Control Plan, Bloodborne Pathogen Exposure Control Plan).

    Demonstrates knowledge of and follows safety practices. Understands the importance of safety, including patient safety in the work place. Maintains a safe environment for self and others.

    Actively participates in the Patient Safety Program, including event reporting. Identifies sentinel events/near misses and responds per defined organization processes. Participates in education activities and process implementation. Demonstrates advocacy for the patient/customer and appropriately acknowledges patients, customers and visitors.
