- Evaluate clients & develop a treatment plan that will meet their needs.
- Choose activities that will help the clients to carry out their daily works independently.
- Monitor clients as they carry out the task given to them.
- Observe & assess clients progress and prepare detailed reports accordingly.
- Maintain safe and clean working environment.
- Maintain documentation of client care services by auditing records.
- Maintain a cooperative relationship among healthcare teams & contributes to team effort.
- Graduate of an accredited Physical Therapy Program.
- Current New York State PT License.
- Experience in Assisted Living settings preferred.
- Proven exceptional customer service and patient care experience.
- Self-motivated with the willingness to exceed patient expectations.
- Ability to work in a fast paced environment while maintaining a positive attitude.
