At Crestwood Medical Center, we believe in the power of people to create great care We look forward to you becoming the next RN Case Manager, Monday - Friday with some weekends 8 hour shifts at our 180 bed full service acute care facility.
We offer a family friendly work environment, perfect for a new grad looking to start their career or an experienced nurse looking to make a changeAs a Registered Nurse Care Manager you will be responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management, and developing a safe appropriate discharge plan in collaboration with the multidisciplinary team.
You will facilitate the progression and transition of care using established criteria in conjunction with the multidisciplinary team.You will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.

Associate Degree in Nursing or Nursing DiplomaBachelor's Degree in Nursing
Required Experience: 2+ years' experience in Care Management or 3+ years' experience in clinical nursing
Required License/Registration/Certification:
Preferred License/Registration/Certification: ACM or CCM Certification
