Program Overview

Fallon Health's Summit ElderCare is a Program of All-Inclusive Care for the Elderly (PACE). PACE is an alternative to nursing home care, helping people 55 and older continue living safely at home.

Job Summary

The SE Licensed Practical Nurse will be part of the Summit ElderCare Care Team, ensuring participants receive high-quality and timely care and support.

Responsibilities

• Implement plans of care formulated by Providers and Primary RNs in collaboration with the Interdisciplinary Team.
• Carry out nursing tasks as assigned by direct supervisors, including medication administration, vital sign recording, and wound care.
• Make frequent home visits to evaluate home environments, fill medication packs, and record vitals.
• Deliver health teaching and make recommendations to the IDT regarding unmet participant needs.

Requirements

• Graduate of an Accredited Practical Nursing Program.
• Valid license to perform Practical Nursing in the Commonwealth of Massachusetts.
• Certification in CPR/Healthcare Provider issued by The American Heart Association or other accredited agency.
• At least three years' nursing experience in a primary care practice or long-term care facility.
• At least one year working in a nursing capacity with frail elders.
