This is an amazing opportunity to be part of Horizon PACE's program for individuals 55 years or older who need coordinated medical care to continue living as independently as possible in their home and community.

Responsibilities
- Consults each day with the Home Care Coordinator (or designee) to be updated on Horizon PACE participant's status, changes in plan of care and to confirm scheduled visits.
- Assists participants with personal hygiene such as skin, mouth, hair care and bathing
