Job Responsibilities:

Providing oversight and support to Case ManagersMonitoring pediatric and adult patient care Managing referrals and assigning cliniciansCoordinating care within the organization and with outside agenciesEvaluating patient outcome scores related to value-based purchasing.

Continuously assessing patient needs and using a team approach to assist patient and family in meeting those needsDeveloping, implementing, and updating plans of careQualities:
