Job summary
This position is an onsite role and does not offer a hybrid or remote option Provides triage, assessment and treatment to patients and familiesResponsibilities Implements counseling/therapy for assigned patients in both individual and group settings Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives Determines provides arranges needed internal external services/interventions Participates arranges case reviews/care conferences to evaluate progress Assists with discharge planning processes including identification community-based resources refers as part of the discharge plan Assesses collaborates coordinates patient's discharge planning needs readiness for discharge with other members of the care team Utilizes appropriate teaching methods referrals ensure patient/family educational needs met Participates trains new staff students interns as assigned Within scope job requires critical thinking skills decisive judgement ability work minimal supervision Must be able to work fast-paced environment take appropriate action
