Job Description
The Licensed Practical Nurse (LPN) is responsible for providing both direct and indirect skilled nursing care to residents. The LPN will follow established policies and procedures and work under the supervision of a Registered Nurse (RN). This role requires a solid understanding of patient care specific to the adult and geriatric populations.
Responsibilities
- Collect and communicate data regarding residents' physical and psychosocial conditions to the RN and interdisciplinary team.
- Contribute to the development of age-appropriate and culturally relevant care plans reflecting nursing diagnoses and interventions.
- Recommend revisions to care plans after evaluating resident outcomes and identify the need for referrals.
- Maintain current knowledge in nursing practice and participate in the mentoring or orientation of staff.
- Ensure safety, cost-effectiveness, and impact in care planning and delivery.
Required Experience / Certifications / Licensure
- New York LPN Licensure
- Basic Life Support (BLS) certification (AHA or RQI only)
- Minimum of one year of experience as an LPN, preferably in a long-term care facility
