This role is perfect for a self-motivated OT who wants to help the geriatric and medically complex populations achieve independence in their own homes.

    Financial Breakdown (Weekly)
    Taxable Pay: $1,280.00

    Non-Taxable Stipends: $1,246.00

    Total Gross Weekly: $2,526.00

    Assignment Overview
    Duration: 13 Weeks | Start Date: ASAP

    Schedule: Monday – Friday (8:00 AM – 5:00 PM)

    Guaranteed Hours: 40 Hours

    Setting: In-Home Care (Reliable transportation required)

    Location: Grand Junction, CO

    Role Highlights
    Full-Cycle Care: Perform comprehensive visits including Evaluations, Start of Care (SOC), reassessments, and discharges.

    Skilled Interventions: Lead ADL/IADL training, upper extremity strengthening, fine motor coordination, and adaptive equipment training.

    Safety Mastery: Implement energy conservation, home safety, and fall-prevention strategies tailored to the patient's unique living environment.

    Technology: Utilize Homecare Homebase (HCHB) for all clinical documentation and care coordination.

    Collaboration: Work with a multi-disciplinary team (RNs, PTs, STs) and provide oversight for OTAs when applicable.

    Mandatory Requirements
    Licensure: Active Colorado Occupational Therapist License (DORA).

    Certification: Current BLS (AHA).

    Experience: Graduation from an accredited OT program; Home Health experience and familiarity with Medicare/OASIS documentation is highly preferred.

    Logistics: Valid driver's license and a reliable vehicle for daily travel.
