- Complete admission, recertification assessments, as well post hospitalization visits.
- Administering medications to patients and monitoring them for side effects and reactions.
- Respond to emergencies in a timely manner, troubleshooting with nurses in the home as well as family members.
- Obtain medical orders from medical doctors, nurse practitioners, physician assistants. Monitoring, reporting, and recording symptoms or changes in patient conditions.
- Administering non-intravenous medications.
- Assist with family complaints and ensure DOH regulations are met.

- Complete LPN/RN/CHHA annual supervisions. Assist clinicians with clinical issues.
- Maintain accurate and timely documentation electronically.
- Ensure all precautions are implemented.
- Bachelor's degree or master's degree in nursing.
- Current state licensure as a registered nurse and BLS certification required.
- American Heart Association BLS/CPR and be ACLS-certified.
- Ability to effectively communicate with patients, families, physicians, and hospital staff.
- Basic computer skills.
- Professional, friendly attitude.
- Experience in-home health care.
- One year of professional experience as a Registered Nurse.
- Experience in home care nursing preferred.
