    Summary



    The Registered Nurse, Home Based Primary Care (HBPC), works with an interdisciplinary Patient Aligned Care Team (PACT) to provide direct patient care to Veterans in their home within the assigned catchment area. The HPBC RN develops and coordinates the patient's plan of care, interacting, coordinating and advocating for the patient/caregiver with other relevant parties, providing individualized education and support and providing intensive case management.

    The Registered Nurse, Home Based Primary Care (HBPC) duties include but are not limited to:

    • Primary responsibilities include case management (advocacy, cultural competence, social determinant awareness, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, care planning, communication, and documentation).
    • Independently plans and delivers technically competent care in the home of Veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, and palliative care.
    • Makes home visits to Veterans in the program. Travel is required. The HBPC RN must maintain a valid and unrestricted state license as home visits are required. The HBPC RN uses personal or government assigned vehicle when traveling to Veteran's home for visits.
    • Utilizes interventions, which maximize health capabilities (i.e., safely administers medications, teaches patients and caregivers to safely administer/take medications, utilizes techniques for management of patient behavior, provides appropriate activities to enhance psycho-social and physical functioning of patient) and reduces unnecessary urgent care and emergency room visits, per veterans' goals of care.
    • Works collaboratively with the interdisciplinary team and other VA and community healthcare entities when planning and providing care to Veterans at home.
    • Demonstrates knowledge and skill in performing nursing procedures applicable to practice area to include but are not limited to: vital signs, pain assessment, venipunctures, appropriate delivery of specimens to lab, accurate filling an/or monitoring of medication dispensers in the home, telephone triage, medication reconciliation, oxygen safety assessment, fall assessment and prevention.
    • Evaluates and documents patient response/progress towards attainment of goals/expected outcomes with minimal guidance. Revises plan of care according to evaluation.
    • Develops and involves patient/family in establishing realistic, measurable goals, planning care, discharge planning, and patient education. Initiates and completes documentation in the medical record to reflect teaching and outcomes.
    • Participates in quality improvement activities in HBPC program and across the facility with the objective of improving Veteran care outcomes.
    • Demonstrates role modeling, leadership and advanced clinical expertise to improve the delivery of patient -centered care through the use of the nursing process for patients requiring specialized/complex care.
    Pay: Competitive salary, regular salary increases, potential for performance awards
