- Visit members with complex care needs at home (6-8 members per day)
- Obtain history and perform physical examination
- Complete review of medicines and related instructions
- Review home safety and psychosocial needs
- Fulfill documentation needs for STAR and HCC measures
- Obtain and document all pertinent information in patient's charts and case management notes on same day as patient is seen; include medications and end of life preferences and drivers of preventable hospital admissions
- Instruct all patients, caregivers and family to call you prior to accessing medical care, subject to life threatening emergencies
- Maintain patient safety and confidentiality
- Universal precautions utilized in all patient care
- Prescribe/order procedures, treatments and medications within scope of practice
- Rotate with the inpatient hospitalist's team for an assigned frequency and duration to be determined by the Campus Medical Director in order to maintain excellent clinical skills
- Evaluate patients at the Post Discharge Clinic with the attending Physician
- Develop high level of trust and intimacy with patients and families
- Required Experience:
- At least 1 year of prior experience
- Graduation from an accredited Nurse Practitioner program
- Board Certification (NP)
