    Overview

    Responsible for creating an exceptional DCH Home Health customer experience. Coordinates business development and education of patients, insurance companies, physicians, hospitals, nurses, community resources, other health care specialists and the local community. Ensures continuity of care, smooth interaction and communication between all involved in patient care activities. In conjunction with the Home Health Director, is responsible for development of home health programs within the communities. Maintains the objectives, goals, and philosophy of healthcare delivery in conjunction with the DCH mission and cornerstones

    Responsibilities

    1. Works independently to generate referrals for home health by building relationships with physicians, long term care, independent and assisted living facilities and other community resources
    2. Participates in patient need identification processes and is accountable for completing the referral process including communicating to the patient, the family, the staff and other professionals as appropriate.
    3. Provides follow-up on patient hospitalizations and makes contact with patients returning from the hospital within 24 hours
    4. Participates in consistent meetings with Home Health Director to review customer experience, quality of care delivery, and program growth.
    5. Serves as an educational resource to the community regarding healthcare services. Coordinates available resources for staff and families. Markets DCH to any appropriate audience.
    6. Facilitates a consistent service delivery of the DCH continuum including highest quality care for the patient and family.
    7. Participates in community DCH program development. In cooperation with other DCH leaders, assesses the needs of the program and develops a marketing plan of action with quarterly goals.
    • Conduct market analysis; develop sales strategy, goals and plans.
    • Conducting sales calls, and evaluating results and effectiveness of sales activity
    • Support business development activities and help establish strong relationships with new and existing referral sources.
    1. Develops and delivers health educational services.
    2. Runs all necessary reports to operate the business. Demonstrates the ability to analyze the data and think critically through problems and solve identified issues to achieve the business objectives.
    3. Maintains performance, patient and employee satisfaction and financial standards as outlined in the performance evaluation.
    4. Performs compliance requirements as outlined in the Employee Handbook
    5. Must adhere to the DCH Behavioral Standards including creating positive relationships with patients/families, coworkers, colleagues and with self.
    6. Requires use of electronic mail, time and attendance software, learning management software and intranet.
    7. Must adhere to all DCH Health System policies and procedures.
    8. May, at times, be required to attend work related functions/ events after hours and on the weekend.
    9. Is responsible for managing the F2F documentation process and assisting with signed order recovery.

    Qualifications

    Must have exceptional organizational, communication and relationship-building skills. Ability to work in stressful situations. Must be able to read, write legibly, speak, and comprehend English.
    • Bachelor's Degree preferred. One year of marketing experience in the health care industry or five years of experience directly dealing with physicians and mid-level providers in a medical setting required.
    • Excellent planning, organization and presentation skills are critical.
    WORKING CONDITIONS

    Is able to lift 20 pounds. Ability to sit for prolonged periods. Able to walk and climb stairs with occasional kneeling, stooping, reaching and required. Good manual and finger dexterity. Must have excellent written and verbal communication skills. Must be licensed and insured to operate a motor vehicle and be able to travel to various locations. Ability to work Monday-Friday 8 am to 5 pm holidays, weekends, and after hours as departmental business requires. Requires use of electronic mail, time and attendance software, learning management software and intranet.

    Physical presence onsite is essential. Hearing and vision must be normal or corrected to within normal range. Able to perform the duties with or without reasonable accommodation.
