Join our team as a Pharmacist at a closed-door, long-term care pharmacy in Albuquerque, New Mexico. This position offers immediate employment with the possibility of a contract extension or permanent placement.

As a Pharmacist, you will be responsible for medication review, dispensing, patient counseling, clinical decision-making, medication management, medication therapy management, drug information, quality assurance, documentation, and interdisciplinary collaboration.

Ideal candidates will have at least 6 months of pharmacy technician experience, with a background in retail pharmacy being particularly suited to these roles.

Duration: 13-weeks (possible contract to hire)

We have an immediate need for a Pharmacist to join a closed-door, long-term care pharmacy in Albuquerque, New Mexico.

This position offers an excellent opportunity for immediate employment with the possibility of a contract extension or permanent placement.

The position offers a variety of shifts to accommodate different schedules.

Job Duties:
1.

Medication Review:
Review medication orders and patient profiles to ensure accuracy, appropriateness, and safety of prescribed medications.

2.

Medication Dispensing:
Prepare and dispense medications accurately, following established protocols and dosage instructions.

3.

Patient Counseling:

Provide counseling to patients on proper medication use, potential side effects, and drug interactions to promote medication adherence and patient safety.

4.

Clinical Decision Making:

Assess patient medication therapy needs, make recommendations for therapy optimization, and collaborate with healthcare providers to develop individualized treatment plans.

5.

Medication Management:
Oversee medication regimens, monitor patient response to therapy, and adjust treatment plans as needed to achieve desired outcomes.

6.

Medication Therapy Management:
Conduct comprehensive medication therapy reviews, identify and resolve medication-related problems, and optimize medication regimens for improved patient outcomes.

7.

Drug Information:

Serve as a resource for healthcare providers and patients by providing accurate and up-to-date drug information and clinical guidance.

8.

Quality Assurance:

Participate in quality improvement initiatives, medication safety programs, and regulatory compliance activities to ensure the highest standards of pharmacy practice.

9.

Documentation:
Maintain accurate and complete medication records, documenting interventions, assessments, and communications in accordance with regulatory requirements.

10.

Interdisciplinary Collaboration:

Collaborate with other healthcare professionals, including physicians, nurses, and pharmacy staff, to coordinate patient care and optimize medication therapy outcomes.

Experience:

Must have at least 6 months of pharmacy technician experience
Retail pharmacy technicians are ideal for these roles

License:
NM pharmacist license required
