The Opportunity Alliance is seeking compassionate, organized and dedicated Care Coordinators to join our team-based case management model called Behavioral Health Home and make a meaningful impact in the lives of our clients.

- Care coordinators meet one on one with clients to assess and support their needs,
- and also have the support of our integrated care team.
