Job summary
The Social Worker will work with the Clinical Care Team to provide social support navigation for social determinants of health (SDOH) such as food insecurity, housing insecurity etc. They will compile and maintain a resource list for SDOH resources including eligibility criteria referral process and contact information.They will also conduct case management activities such as working with hospitals for discharge planning follow-up and education assisting with obtaining patient records from hospitals securing needed medical equipment through community partners conducting follow-up on care plans identifying patients lost to follow-up or overdue for care and assisting them in returning to care.
The Social Worker may assist with specialty referral navigation scheduling coordinating and tracking non-BCS specialist and imaging referrals compiling and maintaining resource lists etc. they may also assist patients in locating accessing low-cost prescription options through patient assistance programs discount retailers etc.
