    Job Description:
    PURPOSE OF JOB:

    To render nursing care, support and instruction to terminally ill patients and their families in order to maximize their comfort. This will be performed with respect to the appropriate specific Chronological Age, Developmental Age or Social Maturity of the Patient.

    II. ESSENTIAL FUNCTIONS:
    • Promotes patient safety through adherence to policies, procedures, guidelines, Joint Commission standards, and state and Medicare regulations. Completes HIS as per policy 90%. Human Resources and Occupational Health requirements completed on time 100% (ex: licensure, RAHE, flu shot,etc.) (20%)
    • Demonstrates excellent clinical competency at all times according to departmental and program standards as well as the standards of each discipline. Home Health Aide Supervision completed 100%. (20%)
    • Demonstrates patient case management by assessing, planning, implementing, and documenting patient care needs according to department guidelines in order to achieve quality outcomes. (20%)
    • Communicates and collaborates with patients, families, physicians, and others to provide coordination of care and customer service. Syncing, documentation compliance 90%. (20%)
    • Demonstrates fiscal responsibility by controlling costs, and enhancing revenue. (10%)
    • Limits special requests for DME/Supplies
    • Authorizations completed and discharges completed according to policy 90%
    • Performs on-call when necessary to meet patient needs (10%)
    Job Qualifications:
    • QUALIFICATIONS:
    A. Education: Registered Professional Nurse with current license with the Bureau of Professional & Occupational Affairs in the Commonwealth of Pennsylvania.

    B. Experience: One year recent medical-surgical experience or community health experience desired. Two years of experience preferred.

    C. Other Skills: Current CPR certification.

    Current drivers' license and auto insurance.

    Ability to drive a car in inclement weather.

    Auto in good working condition.

    Professional attire for patient care.

    Basic Computer skills.

    Hospice nursing must be available for on-call as needed and live within 30 minutes of the hospital.
