    Hospice RN Case Manager Position

    We are seeking a highly skilled and compassionate registered nurse (RN) for a rewarding career in Hospice care at Amedisys, one of the largest and most trusted homecare companies in the U.S.

    Job Summary

    This is a Monday to Friday, 8am-5pm position with occasional on-call duties. The ideal candidate will have one year of nursing experience and be currently licensed as an RN in Pennsylvania.

    Key Responsibilities:

    • Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
    • Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
    • Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness, and according to care center policies.
    • Records pain/symptom management changes/outcomes as appropriate.
    • Teaches patients, families, and caregivers appropriate patient care techniques.
    • Regularly communicates with clinical manager and care team.
    • Performs on-call responsibilities and on-call services to patients/families as assigned.

    The salary for this position is estimated to be between $85,000 and $90,000, plus mileage reimbursement, based on the job requirements and location in Washington, PA.
