Description

The Occupational Therapist is responsible for providing rehabilitative therapy to patients upon referral for cognitive and/or ADL physical functioning disorders following all regulatory and clinical practice requirements.

This includes recovery, improvement in function and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services.


Specific Requirements:

Must hold current and active State license or RegistrationOne year's experience in long-term care or related setting preferredThis position reports to the Director of Rehabilitation Services.1625 E Main Street, Clintonville, WI 54929Our facility follows the Centers for Medicare and Medicaid Services' COVID-19 vaccination requirement.
