    POSITION SUMMARY:

    Responsible for the assessment and treatment of hospitalized patients in keeping with established policies and procedures within Catholic Medical Center. Provides patient education and coordinates treatment with ancillary services within Catholic Medical Center and outside agencies and organizations for the delivery of the highest applicable standard of medical and professional practice.

    Hospitalist will engage in direct patient care activities and medical/clinical practice as the principal component of his/her overall professional responsibility within the Department.

    PRINCIPAL DUTIES AND RESPONSIBILITIES:

    • Admission, assessment, evaluation, diagnostic determination, on-going treatment and clinical management, necessary referral for consultation, care planning and modification, medications/therapeutic orders, transfer authorization, risk assessment and, when appropriate, determination of readiness and planning for discharge for hospitalized patients at Catholic Medical Center.
    • Direct consultation with patients and when permitted, with family members, friends, advocates and others to ensure effective communication of patient status and care plans.
    • Appropriate and on-going communications with ambulatory primary care physicians who routinely manage the care of patients currently in the hospital.
    • Day-to-day rounding /contact/care management that actively encourages and includes input from all members of the patient care team.
    • Accurately and timely completion of all clinical and professional records and documentation to assure optima patient care and compliance with local, state and federal regulatory, licensure and accreditation requirements.
    • Work to create a positive work culture within cross-departmental teams and align this work with other Hospital Medicine initiatives.
    • Perform other related duties incidental to the work described herein.
    • Exhibits positive citizenship by timely attendance, utilization of Kronos, CMC email monitoring and committee participation.
    • Adheres to infection control procedures, safety procedures and Universal Precautions.
    • Ensures confidentiality of employee, legal, client/patient, budget and all company matters.
    • Demonstrates courtesy, respect and caring to patients, families, physicians and coworkers and maintains professional rapport with external parties and organizations.
    • Participates in staff meetings and cooperates with physicians and coworkers to achieve departmental objectives. Reports concerns and problems as they arise.
    • Provides and promotes effective and compassionate care for patients and their families.

    WORK SCHEDULE:

    · Hours/shifts dependent upon program needs and in accordance with Hospital Medicine policy.

    QUALIFICATIONS:

    Education:

    · Graduate of an accredited APRN or Physician Assistant Program.

    Experience:

    · One to two years as a practicing adult nurse practitioner required .

    · Five years or more as a registered nurse with strong medical/surgical background preferred.

    Licensure / Certification:

    · Current registration as an APRN or Physician Assistant-C.

    · ACLS certified. Must maintain ACLS throughout employment.
