- Provide services aligned with the Families First model using a strength-based, non-deficit approach.
- Assist families with concrete services including housing support, transportation, home cleaning, maintenance and repair.
- Deliver skill-building interventions related to parenting, communication, self-advocacy and self-sufficiency.
- Provide short-term counseling and crisis intervention for families experiencing acute challenges.
- Coordinate referrals and services with community partners including counseling providers, medical services, schools, housing resources and utility assistance programs.
- Complete timely and accurate documentation including service plans, progress notes, termination summaries, goal sheets and weekly reports.
- Present cases at termination conferences and recommend appropriate follow-up services.
- Maintain required face-to-face contact with families based on contract requirements and individual family needs.
- Maintain weekly communication with referring workers and complete follow-up evaluations at three- and six-month intervals post-service.
- Transport clients as needed using a personal vehicle.
- Bachelor's degree in social work or a related human services field (required).
- Experience working with children and families preferred.
- Valid Michigan driver's license, vehicle registration and auto insurance required.
- Strong organizational, communication and problem-solving skills.
- Reliable, compassionate and committed to high-quality, family-centered services.
- Full-time schedule with flexible hours and 24-hour availability as needed.
- Hybrid / remote position with home-based work and regular travel throughout the service area.
- Pay rate: $43,000 per year, based on experience.
