    SUMMARY:
    The facility-based APP administers direct patient care services within the Post-Acute Care setting.

    ESSENTIAL DUTIES AND RESPONSIBILITIES:

    • Conduct direct care visits and re-evaluations
    • Evaluate and treat chronic illnesses
    • Review medications and diagnostic tests
    • Coordinate care with collaborating physicians
    • Conduct Annual Wellness Visits
    • Assist with facility-wide illnesses
    • Coordinate care with other disciplines
    • Maintain communication with families
    • Other duties as assigned

    QUALIFICATIONS:

    • Valid nurse practitioner or physician assistant license
    • Masters Degree in related field
    • Strong leadership and interpersonal skills
    • Excellent written and oral communication skills
    • Must have punctuality and organizational skills
