- NYCOur client, a leading healthcare organization in New York City, is seeking a dedicated and experienced RN to join their Home Care team on a full time contract basis.
- Conduct initial assessments and ongoing evaluations of patients' medical, physical, and emotional needs
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Provide skilled nursing care, including medication administration, wound care, and other treatments as needed
- Educate patients and their families on disease management, medication management, and other aspects of selfcare
- Monitor and document patients' progress and communicate any changes or concerns to the interdisciplinary team
- Collaborate with other healthcare professionals to coordinate and ensure continuity of care
- Maintain accurate and timely documentation of all patient care activities
- Adhere to all company policies, procedures, and regulatory requirementsQualifications:
- Active RN license in the state of New York
- Minimum of 2 years of experience in home care, preferably in a float or per diem capacity
- Strong clinical skills, including medication administration and wound care
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexibility to travel to various locations in New York City as needed
- BLS certification
- Knowledge of Medicare and Medicaid regulations is preferredHome Care experience is not required
- If a candidate doesn't have Home Care experience, they must have 1+ years of MedSurg experience
- These candidates will be doing in home visits, primarily wound care/rehab.
- Minumum 1+ years of overall nursing experience in a related field to be considered for this position
- Will not hire new grads/new nurses with less than 12months of experience
- Extensive 2-4 weeks of orientation
- Monday
- Friday schedule 8:30am
- 4:30pm (must also work one weekend per month)
- 5-7 home visits per day
- Must have a car, a NY issued drivers license, and proper auto insurance
